Job Summary
Join our compassionate healthcare team as a Hospice RN Case Manager, where you will play a vital role in delivering personalized end-of-life care that emphasizes dignity, comfort, and quality of life. Duties
- Conduct thorough patient assessments to determine eligibility and develop individualized care plans aligned with hospice standards.
- Manage and coordinate comprehensive symptom control, including pain management and comfort measures, in collaboration with interdisciplinary teams.
- Provide education and emotional support to patients and their families, guiding them through complex medical decisions and end-of-life planning.
- Oversee discharge planning, ensuring smooth transitions from hospital or inpatient settings to home or outpatient hospice services.
- Document all patient interactions accurately in medical records while adhering to HIPAA regulations to protect patient privacy.
- Complete all IDG Comprehensive Assessments, RN Comprehensive assessments, HOPE visits, as assigned- daily.
- Participate in utilization management and diagnostic evaluation processes to ensure appropriate resource use and quality outcomes.
Requirements
- Valid RN license with current registration; experience in critical care settings such as ICU, emergency medicine, or trauma centers highly preferred.
- Demonstrated expertise in hospital experience including medical-surgical units, post-acute care environments, outpatient clinics, or inpatient facilities.
- Strong knowledge of physiology, medical terminology, and patient assessment techniques essential for complex case management.
- Experience working with diverse populations including geriatrics, pediatrics (PICU), behavioral health patients, and those with infectious disease concerns.
- Proficiency in discharge planning, medical records documentation, intake procedures, vital signs monitoring, and utilization management.
- Ability to navigate HIPAA compliance while managing sensitive patient information effectively.
- Excellent communication skills with a focus on patient service and compassionate care delivery.
- Additional experience in managed care settings or outpatient clinic environments is a plus. Pay: $82,000.00 – $85,000.00 per year
