Cedar Valley Community Support Services, Inc. (CVCSS) has an immediate opening for a Program Coordinator for our 24 hour home in Waterloo. This position is responsible for managing the daily services of their assigned members and coordinating services with the member’s interdisciplinary team (IDT), completing accurate and thorough assessments of their members’ strengths and barriers and provide support to achieve program goals. Duties also include being a leader to Direct Care Professional staff through supervision and support of their scheduled times with members to assist the member with achieving their independent goals.
Initially the caseload will consist of 3 individuals with intellectual and developmental disabilities who need 24 hour assistance and supervision in maintaining/obtaining daily living skills enabling them to live as independently as possible in the community. The expectation for this role will include one 16 hour overnight shift per week working directly with the members. There is NO “hands on” care needed – our member’s just need guidance, support, or reminders – they are fully ambulatory and do not require nursing/medical services. As the caseload grows and needs change, the overnight schedule requirement may be adjusted to meet the current need. This role will require hands on training and support to the current DCP staff with building their skills in this role as well, so previous management/supervisory experience is a big plus!
A minimum of 2 years of Human services/social work experience, preferably in HCBS is required. High School diploma or equivalent, a valid driver’s license, current auto insurance and a good driving record is required. All position related/required training is provided.
*Associates/Bachelor’s degree and previous supervisory experience, in a HCBS setting is preferred.
