Job Purpose:
The Outreach Team Leader (OTL) will manage and supervise a team of Community Health Workers and provide community-based services.
Key Responsibilities:
Facilitation of training and mentoring of community health workers
Coordination
Community engagement
Campaigns
Community mapping
Referral and linkage to care
Administration, reporting and document keeping
Screening, preliminary diagnosis and referral
Wellness site services
Qualifications:
Registration with the SANC as an Enrolled Nurse
MUST HAVE: Valid driver’s license and own vehicle
Experience:
A minimum of 5 years appropriate/recognisable nursing experience after registration with SANC as Enrolled Nurse
