Job DescriptionOverviewWe are seeking a dedicated Case Manager / Qualified Mental Health Specialist (QMHS) or CDCA to support individuals receiving behavioral health and substance use disorder services.The Case Manager will work directly with clients to identify needs, coordinate services, address barriers to treatment and recovery, and help clients successfully navigate community resources and support systems. This position works closely with clinical staff and other members of the treatment team to promote continuity of care and positive client outcomes.Candidates must meet the qualifications to serve as a Qualified Mental Health Specialist (QMHS) or CDCA in Ohio.Responsibilities

  • Provide ongoing case management and support to clients receiving behavioral health and substance use disorder services.
  • Complete client needs assessments and assist in identifying barriers related to housing, transportation, employment, benefits, healthcare, treatment, and other social needs.
  • Develop and maintain individualized case management goals in coordination with clients and the treatment team.
  • Connect clients with appropriate community resources, social services, healthcare providers, recovery supports, and other needed services.
  • Coordinate care with therapists, counselors, medical providers, peer supporters, and other members of the interdisciplinary treatment team.
  • Follow up with clients regarding appointments, referrals, treatment recommendations, and identified needs.
  • Assist clients with accessing Medicaid, SNAP, housing resources, employment services, identification documents, and other benefits or community programs when appropriate.
  • Provide education and skill-building to help clients improve independence, stability, and successful community functioning.
  • Maintain professional boundaries and comply with confidentiality and HIPAA requirements.
  • Complete timely, accurate, and compliant documentation of services and client interactions in the electronic health record (EHR).
  • Participate in treatment team meetings, case consultations, and care coordination activities.
  • Maintain communication with internal and external providers to support continuity of care.
  • Perform other case management and client-support responsibilities as assigned.

Qualifications

  • Must meet Ohio requirements to serve as a Qualified Mental Health Specialist (QMHS or CDCA).
  • Experience working in behavioral health, substance use treatment, mental health services, social services, case management, or a related setting preferred.
  • Strong understanding of community resources and social-service systems.
  • Ability to establish professional and supportive relationships with clients while maintaining appropriate boundaries.
  • Strong organizational and time-management skills.
  • Excellent verbal and written communication skills.
  • Ability to work independently while collaborating effectively with a multidisciplinary treatment team.
  • Comfortable working with individuals experiencing mental health, substance use, housing, transportation, and other psychosocial challenges.
  • Ability to maintain accurate and timely clinical documentation.
  • Experience using an EHR system.
  • Valid driver’s license and reliable personal transportation for work-related responsibilities.

ScheduleThis is a full-time, in-person position. Candidates should have flexibility based on client and program needs, including occasional evening or weekend availability when required.Benefits

Pay: $18.00 – $20.00 per hour
